The gap between a village noticing something and a district acting on it

Every disease surveillance system has the same structural weak point: the distance between the moment a health worker in a village or a primary health centre notices something unusual, and the moment a district health officer sees it framed as a decision they need to make. Reports travel upward through a chain of intermediaries. Context thins out along the way — which cluster of cases sits near which water source, which mandal has reported nothing recently and why, which signal is a genuine early warning and which is ordinary variation. A district health officer working with TB surveillance software, or with any district-level disease surveillance tool used in India, often ends up reading spreadsheets and paper summaries that answer "what was reported" but not "what needs attention this week." NETHRA is designed to sit in that gap — not to replace the reporting chain, but to give the people receiving those reports a geographic, prioritised view of what is happening across their jurisdiction.

What geographic signal triage means

Signal triage is the discipline of sorting incoming health signals by where they are, how they cluster, and how urgently they warrant a look — before anyone opens a single case file. A district TB control programme tool, or a broader health department surveillance tool, that treats every report as equally worth a phone call will overwhelm the people using it, and the predictable result is that everything gets skimmed and nothing gets read. Geographic signal triage instead asks a smaller set of questions. Is this an isolated report, or does it sit inside a cluster with other reports nearby? Is it appearing in a location with a known gap in reporting, or a place with steady baseline coverage? Has the same geography raised a signal before, and was that signal resolved or simply left open?

NETHRA's approach is to place incoming signals on a map first and a list second, so that spatial pattern — the thing a human eye catches instantly and a table hides — becomes the starting point for review rather than an afterthought. A list sorted by date tells you what is newest. A map tells you what is next to what. For surveillance work, adjacency is usually the more useful question, and it is the one that conventional reporting formats answer worst.

The four-tier alert triage model

NETHRA organises incoming signals into four tiers, moving from routine awareness to a call for district-level attention. The intent is to reduce every incoming report to one of four clear responses, rather than leaving a surveillance officer to guess how seriously to treat it.

The tiers are about triage support only. NETHRA does not name a condition, interpret an individual case, or tell a district team how many people are affected. It helps them decide where to look first, and in what order, when time and staff attention are the scarcest resources in the room. Every tier assignment is meant to be visible and reversible: a human reviewer can move a signal up or down, and the reason travels with it.

What a surveillance dashboard should show a district officer on a Monday morning

A digital disease surveillance dashboard that district teams can actually use at the start of the week needs to answer a small number of questions immediately, without asking the user to build a query first. NETHRA's dashboard concept is built around that constraint:

The goal is a district TB screening and case-tracking view that a district health officer can absorb at the start of the week and then hand off, review by review, to the right staff — with the handoff recorded rather than remembered.

Outbreak-prevention workflow

Signal triage is only useful if it connects to an actual workflow. NETHRA's design carries a Tier 3 or Tier 4 signal from "flagged" through to "reviewed," "assigned" and "closed," with a record of who looked at it and what was decided. That trail matters for a health department surveillance tool: it lets a state surveillance unit see not only what was flagged, but whether the district acted on it, and how quickly. The orientation is preventive — surfacing a geographic cluster or an unusual pattern early enough that a district team can respond while the response is still small, rather than reconstructing events afterwards from incomplete records.

Closure is treated as a first-class step rather than an afterthought. A signal that is examined and found to be nothing is a useful record: it tells the next reviewer that this geography has already been looked at, by whom, and on what basis. Over time that history is what turns a stream of alerts into a working picture of a district.

What NETHRA deliberately does not do

NETHRA is not an individual patient case-management system, and it is not an official disease-notification channel. It does not issue clinical opinions, and it is not intended to stand between a clinician and a patient. It is a triage and dashboard layer designed to sit alongside existing reporting, adding prioritisation and geography to information that is already being collected. Where a signal needs clinical follow-up, that follow-up belongs to the health system, with NETHRA recording that it was assigned and closed rather than attempting to perform it.

Who this is for

NETHRA is being designed for district health officers who need a surveillance view that prioritises rather than floods; for state surveillance units that need to see triage and response across many districts at once; and for public-health researchers who need clean, geography-tagged signal data in order to study reporting patterns over time. Each of those audiences wants something different from the same underlying record — a shortlist, a comparison, or an exportable history — and the design goal is to serve all three without forcing any of them to read the raw feed.

Current status

NETHRA is a prototype. The four-tier triage model, the geographic dashboard concept and the outbreak-prevention workflow described on this page reflect a design under active development — not a system with completed field deployments or reported outcomes to cite.
